How to Make an Appointment
Business Hours
Our standard business hours, at all locations, are Monday-Friday 9:00 AM - 6:00 PM. We
do have a limited number of earlier, later, and Saturday appointments available
on a case-by-case
basis.
Making an Appointment
Call our Weber County office at (801) 475-0402 or our Box Elder County office
at (435) 257-6999 and we will find a convenient time for both
of us for your appointments. We strive to not make you wait more than 48 hours
for your appointment but this can vary greatly depending on a number of factors.
For example if your insurance company requires that you see a certain
professional, your initial appointment may have to wait longer than 48 hours.
When to Show Up
For your initial appointment you'll need to show up a few minutes early so
that you can complete the required paperwork. For subsequent appointments try to
be on time or a little early.
What to Bring
For your initial appointment you'll need to bring:
- A photo ID
- Your insurance card (if any)
- Your co-pay (or full payment if you have no insurance)
For subsequent appointments, you'll need to bring your co-pay or full payment
if you have no insurance.
Note that we cannot see children under the age of 18 without a parent or
guardian being present in our offices.
How Long Will You Be Here?
The length of your individual sessions will vary depending on your
personal circumstances. Initial appointments are typically 1 1/2 hours long with
subsequent regular appointments typically being 30-50 minutes long.
Group therapy times vary by the group. You can get more information on group
